Items you will need to submit:

       A [site plan] or [survey] that has the location of the dish drawn on it
 

Double check the following:

       Dishes may NOT be mounted on a pole in the front or side yard that is visible from the street.
       Dishes should be at least 20 feet back from the front plane of the house.
       Dishes may not be placed on the front of the home.
